    Northern Queensland Primary Health Network (NQPHN) is a primary health organisation, tasked with the responsibility of coordinating primary health care services for the Mackay, Townsville, Cairns, Cape York Peninsula and Torres Strait regions. NQPHN is an independent not for profit organisations, funded by the Commonwealth Department of Health.
    NQPHN will coordinate primary and preventive healthcare that is, the healthcare that takes place outside of a hospital, such as GPs, allied health, chronic disease management, aged care, mental health and Aboriginal and Torres Strait Islander health. Primary health care is recognised as the most effective way to keep communities and individuals healthy and well.
    The PHN will identify where there are areas of need, such as lack of health care services, difficulty in accessing these services, or regions with particularly high health needs, and work closely with GPs, allied health care providers, hospitals and the broader community to ensure that patients can receive the right care in the right place at the right time.
    NQPHN starts at Dysart, Middlemount, St. Lawrence and Clermont in the south and stretches north to Saibai Island in the outer islands of the Torres Strait, and west beyond Richmond, encompassing Mackay, Townsville, Cairns, Cape York Peninsula and Torres Strait regions.
    ABOUT THE OPPORTUNITY:
    Northern Queensland PHN (NQPHN) is funded to support improved access to after hours primary health care services, and increased effectiveness and efficiency of after hours services, particularly to address gaps in after hours service arrangements and improve service integration within our region.

During the after hours period funded, a pharmacist must be on staff and able to dispense the full range of prescription medication and health advice expected of a fully functioning pharmacy.

    Do you have an innovative idea to improve health outcomes in your region?
    Gippsland PHN is a not for profit primary health organisation with the key objectives of · increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
    · improving co-ordination of care to ensure patients receive the right care in the right place at the right time.
    Two grant funding streams are available:
    1. Expressions of Interest for grants of up to $20,000 to provide seed funding to support the development of community-based health promotion and prevention projects that can be embedded into ongoing practice. Grants will be awarded for innovative projects that are aligned with Gippsland PHN strategic objectives, principles and identified health priorities.
    2. Expressions of Interest of for grants up to $25,000 for innovative solutions from health providers, including but not limited to GPs, allied health providers and pharmacists, to deliver innovative after-hours services that will assist to decrease the rate of primary care presentations to the region's Emergency Departments.
    Projects delivered through these grants must be completed by 30 June 2017 and be able to demonstrate that the seed funding has enabled the project's activities to become part of business as usual activities therefore contributing to health system improvement for Gippsland.

    The Administration Agency will undertake a number of functions in relation to the three programs, which include, the monitoring of the performance of pharmaceutical distributors with regard to their obligations under the CSO Distributor Deed, and CSO Operational Guidelines, and providing compounding payment services.

    Request for Proposals (RFPs) are sought for the development and implementation of innovative initiatives that enhance the capacity of Aboriginal and Islander Community Controlled Health Services (ACHHOs) and/or Alcohol and Other Drugs (AOD) residential rehabilitation centres to address the needs of Aboriginal and Torres Strait Islander communities in relation to AOD and in particular the drug Ice.
    This workforce development model aims to employ an AOD professional who can incorporate AOD service improvement activities into all aspects of policy and practice.
    NQPHN are requesting proposals that can commence from early 2017. The service delivery period will be agreed between the respondent and NQPHN on a case by case basis but is likely to be between 12 months and 24 months.
    Respondents are required to submit proposals that are in line with goals, actions and outcomes of the Alcohol and Other Drugs Service Development model. Refer to the 'Program Framework' in the attached documentation suite for details.

    Under the Sixth Community Pharmacy Agreement (6CPA) Pharmacy Trial Program (PTP) the Department of Health have contracted the Pharmacy Guild of Australia (the Guild) to undertake the trial of a Pharmacy Diabetes Screening Trial in partnership with the University of Sydney and Deakin University.
    The purpose of the PTP is to trial new and expanded community pharmacy programs that seek to improve clinical outcomes for consumers and/or extend the role of pharmacists in the delivery of primary healthcare services. As part of the Trial, the Guild is seeking proposals to supply Point of Care (POC) devices and other consumables to be used by pharmacies participating in the trial.
    You are invited to submit a response to this Request for Proposal (RFP) for the supply "Point of Care (POC) devices for HbA1c POC testing and scBGT POC testing and other consumables to be used by pharmacies participating in the trial".
