Tender Details: The objective of the tendering is to identify a suitably qualified service organisation which is committed to work within an TACP integrated care model programme in partnership with Western NSW Local Health District.
The programme is called Transitional Aged Care Program (TACP) and is a joint Australian government and NSW Ministry of Health program. Capital Health Network (CHN)-Australian Capital Territory Primary Health Network,and COORDINARE-South Eastern New South Wales Primary Health Network, invite Expressions of Interest (EOIs) from providers interested in submitting a Request for Proposal (RFP) to become a lead agency for headspace services in Canberra, Queanbeyan and Bega Valley.
headspace offers early intervention services for 12 to 25 year olds across the key areas of mental health; related physical health; social and vocational support; and alcohol and other drug use.
headspace Canberra and Queanbeyan are currently operated by headspace Services Limited, a subsidiary of headspace National Office. headspace Bega Valley is a new headspace service, funded in the most recent round of headspace services and is yet to be established.
CHN and COORDINARE will be undertaking a formal Request for Proposal (RFP) process to identify lead agencies for these services and are currently developing the relevant documentation. It is anticipated that the RFP will be released in October 2017, accompanied by an industry briefing. The RFP will include service specifications for each of the services and the evaluation criteria.
In the interim, CHN and COORDINARE would like to determine the level of interest in the three headspace services through an EOI process. To express interest, please complete and submit the EOI form to MHSPcommissioning@coordinare.org.au by close of business 27 September 2017. HNECC is pleased to call for submissions for the chance to pitch for one-off funding at our INNOV8 Pitch Night in August, 2017.
A pitch night is an event held for organisations to pitch their innovative ideas to gain exposure and funding. It can best be described as a hybrid of crowd funding and a 'dragon's den' or 'shark tank' pitch event. This round of funding is part of the HNECC PHN's INNOV8 Health Development Initiative.
HNECC PHN is seeking innovative submissions from organisations that are focused on health access/programs that assist the most vulnerable and disadvantaged people/communities.
Proposals will only be considered if they are specific to the HNECC PHN region. Applicants will be required to indicate which sub-region the proposal is for (Hunter, New England, Central Coast), and the locations within these sub regions. Proposals may cover one or more sub-regions.
Our aim is to provide one-off seed funding that will establish innovative ideas that not only meet the needs of the proposed community, but could act as a catalyst that benefits other vulnerable communities. For example, proposals may seek to improve the integration of community services, address gaps in service delivery, drive new technology-enabled care models or improve access to services. This allocation of grant funding is not recurrent.
Proposals that extend beyond June 30, 2018 cannot be funded.
South Eastern Melbourne Primary Health Network (SEMPHN) is seeking tender proposals from suitable suppliers within our region to deliver an Accessible Psychological Interventions (API) service in one or more Local Government Areas (LGAs) within the SEMPHN catchment.

Primary Health Networks (PHNs) were established with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.
The SEMPHN catchment encompasses an area comprising 1.4 million people and stretches from St Kilda to Sorrento and as far east as Bunyip, including the major population hubs of Monash, Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham.

Accessible Psychological Interventions (API) Service
This Request for Proposal (RFP) is an invitation by SEMPHN to suitably qualified suppliers to submit proposals for delivering Accessible Psychological Interventions (API) services within the SEMPHN catchment.
SEMPHN is seeking to engage one or more suppliers to deliver an API service in each LGA within our catchment. Suppliers may apply to provide the API service in more than one LGA.
API services are intended to support consumers with a mental illness who will benefit from the provision of individual, family and/or group psychological interventions from allied health professionals.

Description: With over 3,600 staff and covering an area a quarter the size of Victoria, Bendigo Health Care Group (BHCG) is a 653 bed service that annually:· treats over 42,000 in-patients;
· admits more than 1700 children aged under 16 years to our Child and Adolescent Unit;
· deals with over 50,000 emergency attendances, including almost 11,000 ambulance arrivals to the Emergency Department;

BHCG is seeking quotations for the provision of older persons transitional residential accommodation and psychosocial rehabilitation services (Transitional Services).
The Transitional Services should provide a package of interventions, tailored to individual needs and designed to prevent 'falling out' of society and are designed to facilitate the return to community living for adults and older people with serious mental illness and enduring psychosocial disabilities. These interventions address the following areas:

· Promoting wellness.

Northern Queensland Primary Health Network (NQPHN) is an independent not-for-profit primary health organisation, funded by the Commonwealth Department of Health, tasked with coordinating primary health care services for the Mackay, Townsville, Cairns, Cape York Peninsula and Torres Strait regions.
NQPHN will coordinate primary and preventive healthcare that is, the healthcare that takes place outside of a hospital, such as GPs, allied health, chronic disease management, aged care, mental health and Aboriginal and Torres Strait Islander health. Primary health care is recognised as the most effective way to keep communities and individuals healthy and well.
ABOUT THE OPPORTUNITY:
NQPHN is providing funding to organisations to develop and implement innovative models that meet the After Hours Residential Aged Care Facility needs on the Atherton Tablelands. For the purposes of this proposal, the Atherton Tablelands is defined as those regions within 30 kms of Mareeba or Atherton city centres. The goal of the program is to upskill RACF staff (and other relevant service providers) to deliver comprehensive aged care, thereby assisting in the management of after hours service needs. The program also aims to strengthen coordination and collaborative systems between RACFs and other service providers in the after hours period. An overview of the specific service model available for tender is described in Section 4 of the Service Requirements document. The overview provides a summary of the anticipated program goals, objectives, actions and outcomes. Applicants may address all or parts of the service requirements.
A total maximum funding value of $200,000 (GST exclusive) is available which may be awarded to one (1) or multiple provider/s proposals. Given the scope of the service design, a minimum funding amount for any single application will be set at $50,000 (GST exclusive).

These organisations will be able to demonstrate how their approach will address one or more of the following key themes:
-Improving access to effective End of Life Care services for patients in RACFs and their families, with a focus on after hours
-Building capacity of carers and staff within RACFs to deliver effective EoLC
-Promoting linkages with broader health and support services, to better support integrated/coordinated treatment and referral pathways
REGISTRATION: RFP documents will be available from the Sydney North PHN TenderLink site, once applicants have completed pre-registration on the Sydney North PHN Tenderlink site
INFORMATION SESSION: An information session will be held on Thursday 23rd March at 2.30pm at Sydney North Primary Health Network offices, Level 1, Building 1, 207 Pacific Highway, St Leonards.
