    Support and Transition Service: Suicide Prevention
    This support and transition suicide prevention service will provide non-clinical support to enable individuals to understand and manage their own mental health including accessing the services they need. This service is for any individuals at risk of suicide, or following a suicide attempt, who have:
    * Presented at either Robina or Gold Coast University Hospital emergency departments or;
    * Individuals who have been an inpatient and been discharged from one of these facilities.
    The support and transition service:
    * Demonstrates collaboration between organisations involved in the care of the person and their family and/or carer to provide a support and transition function
    * Improves coordination of care for people who are at risk of suicide
    * Addresses suicide risk at every visit within an organisation, from one health clinician to another or between primary care and health staff in integrated care settings, and;
    * Has mobility among providers, location of care and levels of care.

    ABOUT NQPHN:
    Northern Queensland Primary Health Network (NQPHN) is an independent not-for-profit primary health organisation, funded by the Commonwealth Department of Health, tasked with coordinating primary health care services for the Mackay, Townsville, Cairns, Cape York Peninsula and Torres Strait regions.
    NQPHN will coordinate primary and preventive healthcare that is, the healthcare that takes place outside of a hospital, such as GPs, allied health, chronic disease management, aged care, mental health and Aboriginal and Torres Strait Islander health. Primary health care is recognised as the most effective way to keep communities and individuals healthy and well.
    ABOUT THE OPPORTUNITY:
    Active Healthy NQ grants program aims to improve health outcomes and minimise risk of chronic disease in the Northern Queensland community. The grants program provides funding for health promotion and illness prevention initiatives which aim to change community attitudes and behaviour, and create environments that support people to make healthier choices.
    Round 4 will be a specified grants round targeting maternal and child health, health literacy and Aboriginal and Torres Strait Islander youth health. We are interested in applications which address one or more of the following activity areas:
    1. Maternal and Child Health: develop and implement health promotion activities which focus on preventing use and encourage women to seek support for alcohol, smoking and illicit drug use before and during pregnancy
    2. Health Literacy:develop and implement activities which aim to reduce inequalities in health and improve health literacy
    3. Aboriginal and Torres Strait Islander Youth Health (12 to 24 years): develop and implement health promotion activities which use innovative engagement strategies to empower Aboriginal and Torres Strait Islander youth to improve health outcomes

    This funding will be for support for complex mental health needs for people who live in South Eastern NSW.
    This RFP encourages respondents to suggest different approaches for achieving the desired outcomes, offer new and innovative service delivery methods, focus on the outcomes that are important to people, and be flexible in personalising and localising services where possible. COORDINARE is looking for innovative proposals that:· Align with the geographical distribution of mental health disease prevalence and gaps in current service availability and/or access
    · Involves and includes specific vulnerable groups such as Aboriginal and Torres Strait Islander people and diverse bodies, genders, relationships and sexualities, including those who identify as lesbian, gay, bisexual, transgender and intersex in the whole of system approach
    · Provide links to other services within a stepped care approach to ensure people are matched to a service commensurate with their mental health need.

    OVERVIEW: Sydney North Primary Health Network (SNPHN) is inviting suitably qualified and experienced organisations to apply to deliver innovative and creative approaches to providing Services for young people with, or at risk of, severe mental illness, through a Request for Proposal process.
