    Description: Transition Care provides short-term support and active management of older people with chronic and/or complex care needs at the interface between the hospital and home or with the aged care sectors. Older people can require more time and support when moving from hospital to home or aged care to aid their best recovery and help them remain independent. As at November 2016, there are already 346 TCP places contracted in WA; 296 in the Perth metropolitan area and 50 in regional areas.
    Through this Request, the Department is seeking to engage suitably accredited providers to establish a panel for the provision of Transition Care Services for the Transition Care Program (TCP) within the Perth Metropolitan Area. The required services are delivered within a Residential setting, and within the following Metropolitan Health Service regions:· East Metropolitan Health Service (EMHS);
    · North Metropolitan Health Service (NMHS); and
    · South Metropolitan Health Service (SMHS).
    Respondents may submit an Offer with respect to one (1) or more of the above three (3) specified Health Service regions.
    The Head Agreement established as a result of this Request will constitute a standing offer, whereby the Department, subject to funding and approval, will be able to seek quotes on an 'as required' basis during the Term; enabling effective future demand management.

    Primary Health Tasmania encourages joint applications and collaborative approaches for tender applications from consortia or organisations working in partnership. The service model needs to demonstrate how collaboration and coordination between services providers will be maximised to support service integration and appropriate client referral pathways, including to relevant support services for people with, or at risk of, alcohol or drug misuse issues.
    Successful applicants will be funded from 1 March 2017 and will be offered a service agreement through to 30 June 2019. Key performance indicators will be used to measure services and outcomes.

    Description: With over 3,600 staff and covering an area a quarter the size of Victoria, Bendigo Health Care Group (BHCG) is a 653 bed service that annually:· treats over 42,000 in-patients;
    · admits more than 1700 children aged under 16 years to our Child and Adolescent Unit;
    · deals with over 50,000 emergency attendances, including almost 11,000 ambulance arrivals to the Emergency Department;
    · delivers more than 1,300 babies;
    · conducts almost 82,000 outpatient appointments.
    These services are complemented by a 60-bed rehabilitation unit, ten bed critical care unit and five operating theatres where more than 12,500 surgical procedures are performed annually.
    The organisation provides services in emergency, maternity, women's health, medical imaging, pathology, rehabilitation, community services, residential aged care, psychiatric care, community dental, hospice, palliative care, cardiology, cancer services and renal dialysis to the people of the Loddon Mallee region.
    The three main campuses of BHCG are in Bendigo, with many services extended to regional settings including areas such as Mildura, Echuca, Swan Hill, Kyneton and Castlemaine. Demand on services is increasing rapidly with Bendigo being one of Victoria's fastest growing regional cities. BHCG and the Victorian Government are committed to delivering high quality health-care to the community of Bendigo and the greater Loddon Mallee region, with a substantial investment to deliver a new Bendigo hospital scheduled to open in early 2017.
    When the new hospital opens in early 2017, it will progressively increase the operational capacity over the next five years to include:

    South Eastern Melbourne PHN (SEMPHN) is pleased to invite proposals from organisations to provide low intensity mental health services for the SEMPHN catchment for implementation by Wednesday, 1 March, 2017.
    ABOUT THE TENDERER: SEMPHN was established in July, 2016 and is one of a nationwide network of Primary Health Networks (PHNs) that have been created as part of the Australian Government's current National Health Reforms. PHNs have the key objectives of increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.
    The SEMPHN catchment encompasses an area comprising 1.4 million people and stretches from St Kilda to Sorrento and as far east as Bunyip, including the major population hubs of Monash, Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham.
    As a lead site for primary mental health care, SEMPHN is charged with the development and modelling of innovative approaches to regional planning, integration and stepped care in primary mental health care. This includes innovative approaches to pilot low intensity mental health care interventions, specifically a range of peer-based community support initiatives to support the mental health needs of the region.
    ABOUT THE TENDER: Commissioning Low Intensity Mental Health Services.
    SEMPHN is seeking an interested organisation to deliver via a pilot the following peer-based community support initiatives:
    - Telephone and email based helpline.
    - Peer training, education and support.
    - Peer online forum.
    - Website portal with access to digital resources.
    SEMPHN is seeking to fund an organisation that can demonstrate they are highly skilled in training, engaging and developing peer support workers in the delivery of low intensity mental health interventions, in a nominated region within the SEMPHN catchment from March 1, 2017 to 30 June 2018. The delivery of these initiatives will have a focus on either Dandenong, Frankston or Port Phillip.
    Funding is available to organisations that can demonstrate operational capacity to expand on existing services and pilot these initiatives. Organisations that have expertise in training, engaging and developing peer workers in the delivery of low intensity mental health interventions are encouraged to apply. The total funds available to pilot the peer-based community support initiatives during the contract period in the SEMPHN catchment is $500,000.

    DELIVERY OF PRIMARY MENTAL HEALTH SERVICES FOR YOUNG PEOPLE WITH, OR AT RISK OF, SEVERE AND COMPLEX MENTAL ILLNESS
    Primary Health Tasmania is seeking tender applications from suitably qualified and experienced service providers to deliver primary mental health services for young people with, or at risk of, severe and complex mental illness. The services will aim to improve health outcomes for young people (12-25 years of age) with, or at risk of, severe and complex mental illness.
    Applications can be from individual providers or organisations, or joint applications (e.g. from a coalition, collaboration or consortium). However, the service model needs to demonstrate how client management pathways have been developed in collaboration/partnership with relevant service providers and organisations including the Tasmanian Health Service, particularly Child and Adolescent Mental Health Services (CAMHS) and Adult Community Mental Health Services (ACMHS).
    Successful applicants will be funded from 13 February 2017 and will be offered a service agreement through to 30 June 2018. Key performance indicators will be used to measure services and outcomes.
