    Tender Overview: It is the function of Wide Bay Hospital and Health Service (WBHHS) to establish, manage and deliver high quality public sector health services. To fulfil this function, WBHHS requires quality accessible medical imaging services that are provided in a cost effective and timely manner.
    WBHHS is undertaking a procurement exercise to select a suitably qualified Supplier/s for the provision of radiology services. The Services will be offered in three Lots:
    Lot 1: Onsite radiologist, reporting, interventional, supervision and consultancy services at Bundaberg and Hervey Bay Hospital
    Lot 2: Radiology reporting services to all of WBHHS's facilities including access to 24 hour CT reporting and emergency reporting service for other modalities as required.
    Lot 3: Offsite Radiology and Interventional Services in Bundaberg, Hervey Bay and Maryborough.
    Suppliers may provide these lots completely or partially at one, some or all of the WBHHS medical imaging sites.

    Central Queensland, Wide Bay, Sunshine Coast PHN (the PHN) is a commissioning organisation and is inviting Proposals for initiatives to increase access, efficiency and / or effectiveness of After Hours Primary Health Care.The PHN is one of 31 Commonwealth funded PHNs nationwide. It is a not for profit organisation which coordinates primary and preventative health care for a region of 161,000km2, with a population of 823,985 people; stretching from Glasshouse Mountains in the south, to north of Yeppoon and west of Emerald.
    This opportunity aims to increase the access, efficiency and effectiveness of After Hours Primary Health Care for the communities in the Sunshine Coast sub-region of the PHN. Specifically (but not limited to) those with poor access to Health Services, families with young children (0-4yo), young adults, over 65's and people with mental health issues; and improve access to After Hours Primary Health Care through effective planning, coordination and support for those living with complex and chronic conditions.
    The PHN is seeking Proposals for the development and implementation of innovative After Hours Primary Health Care initiatives, that address gaps in After Hours service arrangements (particularly for priority groups) and improve service integration and continuity of care, for patients with complex and chronic conditions in the Sunshine Coast sub-region of the PHN.
    Eligibility: Proposals are sought from primary care agencies which can include allied health services, community health and community pharmacy, medical support such as imaging / pathology and general practice but does not include in-hospital in-patient services.
    Applications: Applications will only be accepted through this portal.
    Who should complete the application: The application must be completed by an authorised representative of the organisation. A separate submission needs to be completed for each proposal. If partnering with another organisation, the lead organisation should complete the submission.

    The PHN is seeking PreQualification (PQQ) submissions from suitable health service providers interested in developing culturally appropriate service delivery models for Integrated Team Care (ITC) teams. Successful applicants will provide Aboriginal and Torres Strait Islander Outreach Worker and Care Coordination services, in a team care environment, in the Wide Bay area of the PHN for the period 2017-18; with the aim of improving access to general practice services.
    This program is funded by the Australian Government Department of Health.
    The PHN is one of 31 Commonwealth funded PHNs nationwide. It is a not for profit organisation which coordinates primary and preventative health care for a region of 161,000km2, with a population of 823,985 people; stretching from Glasshouse Mountains in the south, to north of Yeppoon and west of Emerald.
    The PHN expectations are for providers to be able to: · Deliver the ITC program in accordance with the ITC Activity Implementation Guidelines;
    · Achieve better outcomes for Aboriginal and Torres Strait Islander people with chronic disease; and
    · Develop flexible approaches to ensure Aboriginal and Torres Strait Islander people are able to access high quality care, including through mainstream practices.
    Applicants may express their interest in delivering the ITC activities in one or more of the PHN's Wide Bay Catchment regions, listed below:
    · North Burnett region (Monto, Mount Perry, Eidsvold, Mundubbera, Biggenden and Gayndah);
    · Bundaberg region (including Childers and Gin Gin); and Discovery Coast region (including Rosedale, Lowmead, Agnes Waters, 1770, Miriam Vale, Baffle Creek and Turkey Beach) these areas are combines as one area for the tender;
    · Fraser Coast region (Hervey Bay, Maryborough, Tiaro, Bauple, Poona, Burrum Heads, Howard and Boompa);
    · OR Whole Wide Bay PHN area.
    The objectives of this PQQ are to:
    · Assess the interest and availability of potential ITC providers;
    · Enable potential providers to outline their proposed solutions to meet the PHN's requirements; and
    · Facilitate an assessment of the PQQs and identify those that best meet the PHN's requirements, to progress to a full tender submission.
    The request for Pre-Qualification submissions is the first stage of a multi stage process. Providers who are shortlisted as part of this PQQ process will be invited to submit a Tender to the PHN as part of the selection process.
    Eligibility: Organisations that can deliver ITC activities and achieve better outcomes, for Aboriginal and Torres Strait Islander people with chronic disease, are encouraged to submit a response.
    Applications: Applications will only be accepted through this portal.
    Who should complete the application: Organisations are encouraged to partner with other organisations when submitting an application. In this instance the 'lead' organisation should submit the application, and reflect the partnership arrangements

    Proposed Term: The proposed term of the contract arising from this multi-stage process is from 01/06/2017 to 30/06/2018.

    Northern Queensland Primary Health Network (NQPHN) is an independent not-for-profit primary health organisation, funded by the Commonwealth Department of Health, tasked with coordinating primary health care services for the Mackay, Townsville, Cairns, Cape York Peninsula and Torres Strait regions.
