    ABOUT THE TENDERER: The South Eastern Melbourne Primary Health Network (SEMPHN) was established in July 2015 and is one of a nationwide network of Primary Health Networks (PHNs) that have been created as part of the Australian Government's current National Health Reforms.
    PHNs have the key objectives of increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time. The SEMPHN catchment comprises 1.4 million people and stretches from St Kilda to Sorrento and as far east as Bunyip, including the major population hubs of Monash, Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham.
    ABOUT THE TENDER: The 3rd Round of After-Hours Grants will provide General Practice in the Local Government Areas of Cardinia, Kingston and Frankston the opportunity to apply for grant funding to increase effective, affordable and accessible face-to-face primary care services in the After Hours period.
    Interested eligible General Practices within these specific LGAs must commit to expanding their services by a minimum of 12 hours during the AH period. At least 2 of the additional hours must occur during a weekend after 12.00 pm on Saturday. Successful applicants must be ready to expand their opening hours by Monday 3 July 2017.

    Description: With over 3,600 staff and covering an area a quarter the size of Victoria, Bendigo Health Care Group (BHCG) is a 653 bed service that annually:· treats over 42,000 in-patients;
    · admits more than 1700 children aged under 16 years to our Child and Adolescent Unit;
    · deals with over 50,000 emergency attendances, including almost 11,000 ambulance arrivals to the Emergency Department;
    · delivers more than 1,300 babies;
    · conducts almost 82,000 outpatient appointments.
    These services are complemented by a 60-bed rehabilitation unit, ten bed critical care unit and five operating theatres where more than 12,500 surgical procedures are performed annually.
    The organisation provides services in emergency, maternity, women's health, medical imaging, pathology, rehabilitation, community services, residential aged care, psychiatric care, community dental, hospice, palliative care, cardiology, cancer services and renal dialysis to the people of the Loddon Mallee region.
    The three main campuses of BHCG are in Bendigo, with many services extended to regional settings including areas such as Mildura, Echuca, Swan Hill, Kyneton and Castlemaine. Demand on services is increasing rapidly with Bendigo being one of Victoria's fastest growing regional cities.     South Eastern Melbourne Primary Health Network (SEMPHN) invites General Practices to apply for a grant to expand their current capacity to improve care coordination for patients with chronic and complex conditions, within the south eastern Melbourne region.
    To be eligible for a grant, your organisation must be able to demonstrate commitment to achieving the following outcomes by the end of the grant period. Your practice will be required to have:
    *effective care coordination strategies in place for people with complex and chronic conditions and;
    *the capability to provide care coordination to a minimum of 30 patients with chronic and complex conditions.
    ABOUT SEMPHN: The South Eastern Melbourne Primary Health Network (SEMPHN) was established in July 2015 and is one of a nationwide network of Primary Health Networks (PHNs) that have been created as part of the Australian Government's current National Health Reforms.
    PHNs have the key objectives of increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time. The SEMPHN catchment comprises 1.4 million people and stretches from Port Melbourne to Sorrento and as far east as Bunyip, including the major population hubs of Monash, Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham.
    ABOUT THE TENDER: This commissioned service is targeted at General Practices to build capacity of their current care coordination service for patients with chronic and complex conditions, within the SEMPHN region.
    By applying eligible organisations:
    1. Commit to contributing to the development and implementation of the work and evaluation plan
    2. Commit to implementing activities to address care for patients with chronic and complex conditions, refer to application guidelines
    3. Deliver activities/ services within the geographical boundaries of the SEMPHN region that reach residents of the SEMPHN catchment.

    Funding grants up to $100,000 each (GST exclusive), will be allocated to General Practices within the SEMPHN catchment on the basis that the activities align with the scope of the commissioned services and the attainment of SEMPHN's expected outcomes.
