MELBOURNE HEALTH. EMR & INTEGRATION PLATFORM INFRASTRUCTURE.
Description: The opportunity:
The Royal Children's Hospital (RCH) is seeking suitably qualified vendors to provide platform infrastructure including servers, storage, software and networking components to extend its current Electronic Medical Record (EMR) system and to implement a new Integration engine system to be shared with Melbourne Health (MH), the Royal Women's Hospital (RWH) and Peter MacCallum Cancer Centre (Peter Mac).
In addition to the provision of platform infrastructure, services will also be required to undertake a detailed infrastructure design, implement and configure the platform infrastructure in accordance with the design and to provide project management.
Hosting: Whilst this ITS seeks the provision of on-site platform infrastructure, as an alternative and as a minimum, vendors responding to this ITS must provide a hosting and infrastructure management option for the provision of the new Integration Engine platform infrastructure. Vendors may elect to provide hosting on site at RCH and Peter Mac to support a co-primary configuration or may propose "cloud based" hosting alternatives.
Also, an optional opportunity exists for qualified vendors to propose the replacement of the existing EMR infrastructure at RCH with new infrastructure and to also provide external hosting and infrastructure management services. Vendors electing to provide a hosting option must provide an "on-site" option as well as a "cloud based" hosting alternative.
Converged Infrastructure: It is recognised that across the requirements within this ITS, that there may be opportunities for a converged or consolidated infrastructure solution. In addition to a conforming response, vendors may also elect to propose alternative converged or consolidated infrastructure solutions.

MURRAY PHN. INTEGRATED MODEL OF CARE WITHIN GENERAL PRACTICE.

Murray PHN has an opportunity for General Practice (GP) to initiate a new model of integrated care for patients with co-occurring conditions of AOD substance misuse and mental health. One proposal per Murray PHN region will be funded along with one Aboriginal Community Controlled Health Organisation proposal.
General Practices are invited to submit an Expression of Interest (EOI) proposal which meets the following criteria:
a) Evidence of location need which includes· Lack of current access
· No established AOD pathways
· Evidence of need;
b) Show integration with the public AOD Treatment Sector (Intake, Assessment, Counselling, Care and Recovery and withdrawal services and mental health systems)
· Illustrate model through diagram
· Show Practice referral pathways;
c) Patients must be bulk billed;
d) Consideration of family friendly practice for this patient cohort.
General Practices will show a plan of:
· what AOD screening tools will be utilized;
· how competence of staff is strengthened
· how the model will be sustained post this funded proposal.
These proposals would have a budget of $50,000 each.
Proposals are to be conducted from July 2018 to December 2018 with some consideration of extension if required.

Gippsland PHN is a not for profit primary health organisation with the key objectives of: · increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and
· improving coordination of care to ensure patients receive the right care in the right place at the right time.
Proposals are sought from suitable organisations to deliver a clinical liaison program, a component of the LifeSpan Model of Service Integration being implemented by Gippsland PHN as part of the place based suicide prevention trials in the Latrobe Valley and Bass Coast.
Total indicative funding for the project is $130,000 (ex GST), for implementation and delivery of services until 30 June 2019.
