Contacts: Jed Fraser, Type: Technical, phone: (03) 9096-2716, email: jed.fraser@dhhs.vic.gov.au
Description: The department is seeking suitably qualified and experienced parties to undertake the South Initiative Evaluation. The South Initiative is a coordinated and contained test of new and innovative service responses for children and young people in out of home care. The South Initiative has two objectives:· To enable government and non-government stakeholders to work together to design, test and refine approaches to out-of-home care that integrate and embed out-of-home care prevention, diversion and transition responses within the broader service system.
    · To examine if this approach to an enhanced home-based care system, coupled with a suite of contemporary treatment responses and flexible, individualised prevention, diversion and transition services can reduce the need for traditional residential care services and improve the experience and outcomes for clients.
The Initiative comprises nine activities underpinned by a range of measures intended to support effective implementation.
The South Initiative will be implemented through an action learning approach involving co-design, testing and refinement of services within a defined geographic location. This will provide evidence to inform the future state wide roll out of out of home care reform.

    SEMPHN-751032
    Notice Type: Request for Tender
    Price Range: Not Specified
    Region: Victoria
    Closing Date: 30/01/2017

South Eastern Melbourne (SEMPHN) is seeking proposals from interested organisations or persons to improve childhood immunisation rates, particularly within identified 'hotspots' in the SEMPHN catchment. Projects are expected to commence on Monday, 3rd April 2017.
ABOUT THE TENDERER: SEMPHN was established in July 2016 and is one of a nationwide network of Primary Health Networks (PHNs) that have been created as part of the Australian Government's current National Health Reforms. PHN's have the key objectives of increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes and improving co-ordination of care to ensure patients receive the right care, in the right place, at the right time. The SEMPHN catchment encompasses an area compromising 1.4 million people and stretches from Port Melbourne to Sorrento and as far east as Bunyip. This includes the major population hubs of Dandenong, Moorabbin, Caulfield, Cranbourne, Frankston and Pakenham. The PHN's activities are guided by a number of priority areas, which include improving childhood immunisation rates. As such, this proposal seeks suitably qualified and experienced organisations/persons who can support immunisation providers in the delivery of services within 'hotspot' areas.
ABOUT THE TENDER Commissioning services to improve childhood immunisation rates within the south eastern Melbourne region.
Within this project, childhood immunisation refers to children within the age groups of: 12 to less than 15 months old, 24 to less than 27 months old, and 60 to less than 63 months old, who are not fully immunised.
SEMPHN has identified areas that are 'hotpsots', which are characterised by communities or practices with low rates of immunisation, or high numbers of children not fully immunised. The following municipalities were identified as hotspot areas within the SEMPHN catchment: Casey, Dandenong, Port Phillip and Stonnington.
Submissions from interested applicants must propose an activity, or a range of activities, that:

    - focus on one or more SEMPHN hotspot areas (whether across regions or the entire SEMPHN catchment); and

are provider-based interventions, such as education for health providers, recall/reminder systems and/or catch ups.
