    SOUTH METROPOLITAN HEALTH SERVICE. FREMANTLE STREET DOCTOR SUSTAINABLE DELIVERY MODEL AND MOBILE HEALTH CLINIC SERVICES.
    Issued by: South Metropolitan Health Service.
    Draft For Comment Number: SMHS2016609Draft.
    Enquiries: Sue Gardner. Phone number: 6152-8082. Email address: sue.gardner@health.wa.gov.au or SMHSTenderBox@health.wa.gov.au
    Description: DRAFT - FOR COMMENT ONLY NO LATER THAN 24 JANUARY 2017.
    SMHS seeking feedback from industry to improve the Application Guideline and Response document prior to release for submissions.
    The aim of the Fremantle Street Doctor Pilot Sustainable Delivery Model and Mobile Health Clinic Services (FSDMHC) Grant is to provide a free, (Medicare bulk billed, no charge to patients) visible, easily accessible, culturally appropriate and non-judgemental mobile medical service for members of street-based and disadvantaged populations in the South Metropolitan Health Service (SMHS) catchment of Fremantle and surrounding areas. The aim is also that services are delivered in a sensitive, confidential and caring manner by the FSDMHC and offer flexible and local service delivery at pre-determined sites encompassing all aspects of clients' physical, mental and social health to a client base who would otherwise not seek care at GP Practices. FSDMHC clients are referred to suitable GP Practices and other providers where possible for ongoing long-term management of health issues.
    The Grant specifications will have a dual purpose for the duration of the Grant period:
    1. Develop Sustainable Street Doctor Service Delivery Model (SSDSDM) - For the market to develop and pilot a sustainable service delivery model for FSDMHC Service, to cease/reduce reliance on State funds.
    2. FSDMHC Service - To fund the financial shortfall between the Medicare rebate and the cost of providing the FSDMHC service to Fremantle and surrounding areas within the SMHS catchment area.

    Description: Transition Care provides short-term support and active management of older people with chronic and/or complex care needs at the interface between the hospital and home or with the aged care sectors. Older people can require more time and support when moving from hospital to home or aged care to aid their best recovery and help them remain independent.     Through this Request, the Department is seeking to engage suitably accredited providers to establish a panel for the provision of Transition Care Services for the Transition Care Program (TCP) within the Perth Metropolitan Area. The required services are delivered within a Residential setting, and within the following Metropolitan Health Service regions:· East Metropolitan Health Service (EMHS);

    · South Metropolitan Health Service (SMHS).
    Respondents may submit an Offer with respect to one (1) or more of the above three (3) specified Health Service regions.
